I had the same issue with my 21 TRD sport at 19k miles. They ended up finding “excessive scaring” in multiple cylinder heads. I ended up getting a new short block under warranty. The whole ordeal took a month and the ticket was $18k. I traded the truck in 2 days after I got it back.

I recently purchased a 2022 Tacoma and when I left for work the next morning the dreaded P11EC code came on. Lucky for me the truck had 59,450 miles so it was still inside its powertrain warranty and it’s being repaired by Toyota right now. I’ve had the loaner longer than I’ve had possession of my “new” truck.
Just had my 2020 Limited Tacoma threw an error code, after 6 hours of diagnostics it was a bad manufactured cylinder head. Lucky for me, I'm 4 months past the warranty (only 48k miles though instead of the 60k on the PT warranty), so now Toyota WILL NOT help at all.
My 1996 Super Custom has over 400k on the clock, had to do the head last year thanks to worn out injectors and a dead fan clutch.
